Fiber is one of the few nutrition recommendations that survives contact with the evidence more or less intact. Which is why the failure mode is not neglect. It is enthusiasm.
The targets
The Institute of Medicine Dietary Reference Intakes, restated in the Academy of Nutrition and Dietetics position paper, put the daily target at 25g for adult women and 38g for adult men, or 14g per 1,000 calories consumed. On a GLP-1, the second formulation is the more useful one. Intake falls, so the absolute target falls with it, and hitting even the middle of the range takes intention rather than volume.
Why the ceiling drops
GLP-1 medications slow gastric emptying, a documented effect in the labelling. Food stays in the stomach longer, which is a large part of how the appetite effect is produced. The same mechanism means a large fiber load now sits and ferments for longer before moving on, producing more gas and bloating than the identical amount would have before treatment.
There is a second, quieter cost. Fiber is filling, and stomach capacity is already reduced. Load up on it and you can fill the available space before taking in enough protein, which puts this directly at odds with the lean mass problem described in Field Note 001. Two pieces of individually sound advice, competing for the same limited intake.
In rare cases a substantially slowed gut combined with a large volume of coarse, fibrous food can contribute to a blockage. This is uncommon, and worth naming precisely because the “more fiber” instruction is given so freely.
Signs the ceiling has been passed
Bloating, excess gas, cramping, feeling uncomfortably full for hours, and (counterintuitively) constipation, particularly where fiber was added quickly or without enough water. At very high intakes fiber can also interfere with the absorption of some minerals.
The absence of enough fiber looks different: hard, infrequent or difficult bowel movements, and a sluggish, backed-up feeling.
What to reach for
Beans and lentils, which carry fiber and protein together and therefore resolve rather than worsen the competition described above. Whole grains such as oats, barley and quinoa. Fruit such as berries and pears with the skin on. A wide range of vegetables. Seeds like chia and ground flax. Against refined carbohydrates, which spike quickly and bring little fiber with them.
What we do with it
The instruction is not “more fiber.” It is adequate fiber, reached gradually, with water, and never at the expense of the protein target. Where a programme is issuing generic high-fiber advice to patients on a GLP-1 without accounting for slowed gastric emptying, it is generating avoidable side effects and blaming them on the medication.
We would also note the evidence quality here is thinner than the confidence with which fiber advice is usually delivered. The targets above are consensus guidance, not trial outputs.
